The HOMEfit-programme lasts twelve weeks and includes a multidimensional home-based exercise programme as well as consultation sessions with an exercise therapist in the general practitioner’s practice and by phone.

The home-based exercise programme consists of strength, balance and flexibility exercises, and brisk walking. During face-to-face consultations, it is adapted to the patient’s individual skills. Consultations by phone serve to motivate participants and to clarify questions and problems. Additionally, impediments to performing the programme as well as strategies for overcoming these individual barriers are discussed.

The autonomous performance of the home-based exercise programme is a basic precondition for the success of the intervention. The programme thus integrates methods and strategies fostering behavioural change.
